Your Name, Degrees Licenses: Clay Smith, RN Your Business Name (if applicable): Sheetclamps.com LLC Business Organization? Limited Liability Company Florida Why did you choose this type of organization? There are different reasons including the fact that an LLC protects the owner from the debts of and any judgments against the business. It's the preferred form of business organization for health care products. Please describe your business/services you provide: We invented the device now known as the Sheet Clampä stretcher sheet fastener. This small inexpensive, unbreakable and reusable device will greatly benefit nurses and patients alike. Simply drape the sheet over the stretcher bar at the top of the stretcher and snap this device over the sheet onto the bar. (See a demo video at www.SheetClamps.com ) The sheet will no longer slide down towards the foot of the stretcher. The result is less work for the nurse and a much more enjoyable experience the client. No one likes the feel of cold plastic on their bare back or the obvious infection control issues involved. Street Address: PO box 118 City: Edgewater State: Florida Zip: 32132 E-mail Address: sheetclamps@jocularity.com Website: www.SheetClamps.com Type of business categories would you would like to be listed (no maximum): Inventors (Nurse) Nurse-Owned Businesses Nurse Inventor Contact person(s): Clay Smith Phone number: 800-830-4835 Start date 08/08/2005 What Attracted You To The Field of Nursing? I Love science. What is your experience and background in Nursing? Emergency Room (ER) How did you hear about the Nursefriendly, NursingEntrepreneurs.com?: yahoo.com What year were you first licensed?1999 How many years have you/did you practice as a nurse: 7 years What made you decide to start your own business? Nursing is greatly rewarding field however, there is a limit to ones potential as a staff nurse! How many times have you changed employers in your career? 7 What specific qualities and traits do you feel make nurses especially qualified to operate a business? If it is the medical field, we have the technical know how and experience that others do not. For instance my invention most likely would only have been invented by a medical person who was acutely aware of the need for just such a device. Do you currently practice as a nurse? YES If yes in what type of setting? Emergency Room (ER) Your most pressing concerns with the profession and healthcare: Healthcare in the USA has realy gotten out of hand, very expensive. I see a disturbing trend among hospitals that require more and more from nurses and they require nurses to get it all done in the same 12 hours as before. It seems to be all about moving patients along as fast as possible with little or no care as to what the patient thinks or continuity of care. Other areas of interest and expertise: I love to invent.

Should You Incorporate Your Registry Business? By Joseph Caracci, RN:"The short answer is yes! When starting a new nurse registry business, you are entering into a legal business environment that requires you to take steps to protect your personal assets, such as your home, your savings, and your retirement. With this in mind, you need to determine which legal form you are going to take for your new registry. Some of the common choices are a sole proprietorship, a partnership, a limited liability partnership (LLP), a limited liability company (LLC), and a corporation." ****************************************************** ****************************************************** The #1 question, we've been receiving lately is about Starting A Nursing Agency, resource sites & manuals

Nurses' Station:"The idea for the Nurses' Station Catalog was conceived in 1989. After searching the marketplace in response to customer inquiries, it became obvious that there were no catalogs of this type serving the nursing profession. To be sure, there were several catalogs offering nurse's uniforms and a smattering of professional items. But there weren't any catalogs at the time offering a range of gifts, clothing, professional items, name badges, shoes and scrubs for nurses. It took two years of hard work to gather samples and put a together a catalog of the most unique and high-quality items for nurses." ****************************************************** ******************************************************
