The capsules are formulated for oral administration with a combination of amlodipine besylate equivalent to 2.5 mg or 5 mg of amlodipine and 10 mg or 20 mg of benazepril hydrochloride. The inactive ingredients of the capsules are calcium phosphate, cellulose compounds, colloidal silicon dioxide, crospovidone, gelatin, hydrogenated castor oil, iron oxides, lactose, magnesium stearate, polysorbate 80, silicon dioxide, sodium lauryl sulfate, sodium starch glycolate, starch, talc, and titanium dioxide."

The combination product is able to increase compliance and, because lower doses are needed, is an ideal combination for geriatric people, small frail individuals, and people with liver insufficiency. Containing two ingredients, Lotrel works in two different ways. The benazepril inhibits an enzyme, ACE, which is involved in the angiotensin-renin system. This system plays an integral role in blood pressure control, and with ACE inhibition blood pressure is lowered. Blood vessels have calcium channels or "pathways" in their walls. Calcium moves along these channels being involved in blood vessel constriction. The calcium channel blocker, amlodipine, affects how calcium moves along these channels resulting in relaxed blood vessel walls and reduced blood pressure."

Special Concerns: Discontinue ACE inhibitors as soon as pregnancy is determined. Use with caution in those with severe renal disease, in CHF, and in severe hepatic impairment. Safety and efficacy have not been determined in children."

Tell your doctor of any over-the-counter or prescription medication you may take including any heart medications, potassium supplements, salt substitutes or any product for cough, cold or allergies. Avoid grapefruit juice while using this medication, unless your doctor instructs you otherwise. Do not start or stop any medicine without doctor or pharmacist approval."

Check with your doctor if you notice any signs of fever, sore throat, or chills. These could be symptoms of an infection resulting from low white blood cell counts. Check with your doctor if you notice difficult breathing or swelling of the face, arms, or legs. These could be symptoms of a serious allergic reaction. Check with your doctor if you become sick while taking this medicine, especially with severe or continuing vomiting or diarrhea. These conditions may cause you to lose too much water, possibly resulting in low blood pressure. Dizziness, lightheadedness, or fainting may also occur if you exercise or if the weather is hot. Heavy sweating can cause loss of too much water and result in low blood pressure. Use extra care during exercise or hot weather. In some patients, tenderness, swelling, or bleeding of the gums may appear soon after treatment with this medicine is started. Brushing and flossing your teeth carefully and regularly and massaging your gums may help prevent this. See your dentist regularly to have your teeth cleaned. Check with your medical doctor or dentist if you have any questions about how to take care of your teeth and gums, or if you notice any tenderness, swelling, or bleeding of your gums. Before having any kind of surgery (including dental surgery) or emergency treatment, tell the medical doctor or dentist in charge that you are taking this medicine."

Amlodipine and benazepril is in the FDA pregnancy category D. This means that amlodipine and benazepril will harm an unborn baby. When used during the second and third trimesters of pregnancy, ACE inhibitors (benazepril) can cause injury and even death of the developing fetus. Do not take this medication without first talking to your doctor if you are pregnant or planning a pregnancy. It is also not known whether amlodipine and benazepril passes into breast milk. Do not take this medication without first talking to your doctor if you are breast-feeding a baby."

Older Adults- This medicine has been tested in patients 65 years of age or older and has not been shown to cause different side effects or problems in older people than it does in younger adults. However, blood levels of amlodipine may be increased in the elderly and elderly people may be more sensitive to the effects of this medicine. Racial differences- Black patients may be less sensitive to the blood pressure-lowering effects of this medicine. In addition, the risk of a serious allergic reaction involving swelling of the face, mouth, hands, or feet may be increased."
